Standard Operating Guidelines Section 14. INCIDENT REPORTS 1. All incidents shall require a report to be completed. Any PVFD personnel completing the report are required to fill in all applicable information (date, run times, service number, address, and location of incident if different than address) and as much information as possible. This report must be legible and accurate to the best of their ability. Narratives will be detailed and explain what the situation was, action taken, and final result. 2. The same applies for EMS reports which shall include patient information, vitals, treatment, and medical supplies used. 3. Reports will have the PVFD member filling out report printed name and signed name. 4. Reports will have the PVFD officer in charge printed name. 5. Reports will have PVFD medical person ID # that treated patient. 6. Reports will have driver of apparatus. Remember: This is a legal document. |
